A finding of substandard quality of care leads to certain notifications. The facility must provide the SA with a list of each resident in the facility subject to the substandard quality of care & the name & address of his/her attending physician, regardless of whether payment is made through Medicare, Medicaid, or private pay. The facility must submit this information to the SA no later than 10 working days after receiving a notice of substandard quality of care.
