For nursing homes, the SA provides the state LTC ombudsman with the following information:
- Statement of deficiencies
- List of isolated deficiencies that cause no harm with the potential for minimal harm
- Report of adverse actions imposed
- Provider's written response
- Provider's request for appeal & results of an appeal
