Background (Face Sheet) Information at Admission Form

The Background (Face Sheet) Information at Admission Form contains:

This information is to be completed at the time of the resident's initial admission to the nursing facility. A new Face Sheet is also required to be completed, along with an Initial Admission assessment, for an individual who returns to the facility after a discharge in which return was not anticipated. CMS's clinical policies, as well as data specifications, allow Face Sheet information to be updated & submitted after the Admission assessment is completed & transmitted. This means that Face Sheet information can be transmitted with any of the Federally required records (those indicated by codes under AA8a) or the assessments required for Medicare PPS (those indicated by the codes under AA8b). The only instance in which Face Sheet information cannot be updated is from those assessments required by the State (AA8a = "0" & AA8b = "6").
